The Western New York Alliance for Person-Centered Care (WNYAPCC) is the first community-wide initiative of its kind in the U.S. aimed at transforming the way nursing homes and senior care providers deliver care to elders. Endorsed by the Centers for Medicare Services, person-centered care is a revolutionary approach to long-term care and assisted living entities that is being adopted by senior care providers across the nation. This initiative, launched in 2008, is made possible by a grant from The John R. Oishei Foundation, Buffalo, N.Y.\r\n\r\nPerson-centered care is a journey that moves decision-making directly to the elders and their care partners, integrating all aspects of daily life in which elders can truly thrive and grow as individuals.\r\n\r\nUltimately, person-centered care is focused on creating a true home environment that makes life worth living.
